Instructions for obtaining access to Online Resuscitation Resource

1. Please print off two copies of the Online Resuscitation Resource Licence Agreement
2. Fill in the blank spaces with the appropriate information:
   • your organisation’s or your name on the front page.
   • your organisation’s or your name and registered address on page 2
   • the number of users requested in the space provided at clause 3. Please note that we require a minimum of 100 users per licence.
3. Arrange for two copies of the licence agreement to be signed by an authorised signatory on behalf of your organisation or sign it yourself if you are signing as an individual.
4. Please send both copies of the signed agreement for acceptance to:
   
   Director - Research, Business and Innovation,
   University of the West of England, Bristol,
   Frenchay Campus,
   Coldharbour Lane,
   Bristol BS16 1QY
   A top copy of authorised licences will be returned to you.
   
5. Methods of payment:
   
   

   Cheque

   A cheque for the appropriate amount (£6.50 plus VAT per user) can be sent with the copies of the licence agreement. These should be made payable to ‘University of the West of England, Bristol’

   
   Alternatively an invoice can be raised by UWE for the required amount and sent to you. This invoice can be paid by BACS, cheque or credit/debit card. If you wish to be invoiced please advise us to who the invoice should be addressed. Please note that your institution must request a minimum of ten users if you would like an invoice to be raised.
   
   
6. To enable us to send usernames and passwords to each user, please ensure that a list containing a valid list of user’s email addresses is sent with the signed copies of the licence agreement.
